You don’t have to be receiving a social service from us to join the scheme. You can apply if:

You live in Rhondda Cynon Taf and are aged between 16 and 50

You have a permanent disability or chronic illness

You are eligible to receive this service

You are comfortable with the idea of taking part in group activities that are agreed by the members

All about the scheme

We’ll assess you for inclusion on the scheme. The assessment will help us to find out if you’re eligible to receive the service and become a member of the group

It’s geared to your needs and you choose, with the group, what you want to do

You’ll be supported by the rest of the members of the group

There’s a varied programme of things to do, usually locally, sometimes further afield

It gives you the chance to make friends and socialise
