The Health Social Care and Well-being Partnership (which includes Rhondda Cynon Taf County Borough Council, Rhondda Cynon Taff Teaching Local Health Board, Cwm Taf NHS Trust, the Voluntary Sector and other key stakeholders) has recently finalised its Health Social Care & Well-being Strategy for 2008-11. This builds on the achievements of the first Strategy which was operational from 2005-2008 and aims to tackle some of the underlying causes of ill health in Rhondda Cynon Taff as well as addressing the health inequalities which exist in many of the deprived areas in the Borough.

Condition Management Programme

The Condition Management Programme is a new health provision under the Government’s Incapacity Benefit Reforms and was established in March 2004 as a result of the Green Paper “Pathways to Work: Helping People into employment.
 
The philosophy of the Condition Management Programme is on the prevention and management of ill health and aims to re-focus incapacity on capacity. 
 
The service aims to help individuals to manage their health condition more effectively by educating, advising and reassuring them about the management of their condition on a day-to-day basis so that they feel more confident about returning to work. The service covers three main healthcare categories:
 
  • Moderate Mental Health Conditions
  • Musculoskeletal conditions
  • Circulatory and respiratory conditions.
The team work with clients either on a one-to-one basis or in groups. The range of groups activities being delivered by the team now totals six and feedback from both clients who have attended the programme has  been extremely positive. The group activities include:-
 
  •  PainPac                Pain management group
  •  RelaxMax             Relaxation Group
  •  Focus Forward     Work & Health focused group addressing next steps to employment
  •   FearFacer             Anxiety management group
  •   Moodmanager     CBT based depression management group
  •   Stepping Out       Social engagement group

The service is being delivered in communities across the Rhondda Cynon Taff/Bridgend district by a team of dedicated NHS practitioners comprising of occupational therapists, physiotherapists, cognitive behavioural practitioners and a general practitioner.
